Cognitive behavior therapy is done in the natural setting. A fear hierarchy, which rank orders situations that cause distress from least to most is constructed. Coping skills are taught to help reduce anxiety. Desensitization/gradual exposure begins with exposure to situations that cause little or no anxiety. Once distress habituates and disappears (due to exposure) in a situation, desensitization/exposure progresses to the next situation on the fear hierarchy. Fading and generalization are used to minimize distress when introducing new situations. With children, consultation and coordination with schools, the family and the community is done to facilitate the development of a comprehensive program.
